DepoDur Ordering Information

*Setting Up Your Account

To order DepoDur for the first time, an account must be set up with EKR's Distribution Center - the sole distributor for DepoDur. Follow these simple steps:

Step 1: Complete the linked Account Setup (KYC form), for DEA Controlled Substances or call (877)-757-0662.

Step 2: Fax the completed KYC form, a copy of sites DEA Registration, State Board of Pharmacy License, and State Controlled Substance License (if required) to customer service at 877-204-4154.

Ordering DepoDur

After the account setup documents have been faxed you can order DepoDur (drop shipment only), as follows:

Step 1: Complete an original DEA Form-222 and mail it to EKR's Distribution Center using the EKR UPS account number

To expedite processing, you can fax the DEA Form-222 to 877-204-4154 in addition to mailing the hard copy, but your order will not be shipped until the original DEA Form-222 has been received.

Step 2: Please be sure to fill out the DEA Form-222 correctly (DEA Form-222 SAMPLE)

*The first time DepoDur is ordered follow the steps for "Setting Up Your Account" and "Ordering DepoDur." For subsequent orders of DepoDur, you will only need to send an original DEA Form-222.

Receiving Your Shipment

Upon EKR's Distribution Center receiving all required forms:

	The EKR Distribution Center will contact the hospital's preferred wholesaler to obtain a purchase order number.
	After order approval, the EKR Distribution Center will only drop ship DepoDur to the hospital via next-day delivery. (DepoDur will ship Monday through Thursday.)
	EKR Customer Service will invoice the wholesaler at the hospital-contracted price. The wholesaler will invoice the hospital at the hospital-contracted price. EKR does not accept chargeback's.
